Door to Door Reservation Request

Reservations may be requested up to 7 days in advance and must be submitted no later than 4:00 PM for next day travel. GoCary does not accept “same day” reservations. Registered Door to Door customers may request a reservation using this form, or by calling (919) 481‐2020, Option 3.

This form is for one or more trips on the same day.  If you would like to make reservations for multiple days, please complete a separate form for each date.

Are you traveling with a Personal Care Attendant (PCA)?
This option is only available to customers that have been pre‐approved to travel with a PCA.

If your destination is in Tier 3 (Apex, Morrisville, Raleigh, Durham or Chapel Hill), are you traveling for work or medical reasons?
